Please fill up this form if you wish to attend the concert. Please note that the venue has a limited capacity and if the number of applications exceeds the capacity, attendees would be chose at random.

Time: 6.30pm (doors open), 7.30pm (concert starts)

Venue: Zouk KL, Jalan Tun Razak

- We will be asking you to show us your photo ID (IC, driving license or passport) when you are registering at the venue for the concert. Please ensure that the name you enter in this form is the same as the photo ID you will be presenting. If the name is not the same, you will not be allowed to collect your entrance proof.

- By registering for this event, you understand and consent that your image and likeness may be taken in photo and/or videos, and would be used for marketing and promotional purposes.

- All details asked for in the application form will only be used for onsite verification purposes to ensure that the correct person gets the entrance proof. All personal information collected will be kept with the strictest confidence and will be deleted after the event.
